Memory Support & Advisory Service

Memory Support and Advisory Service was launched in September 2014. It is one initiative that can help local people and their families with memory loss or a diagnosis of dementia. Figures from NHS England suggest that there may be up to 13,312 people in Dorset living with dementia, 300,000 nationally, who do not have a diagnosis. This makes increasing dementia diagnosis rates a key priority for NHS Dorset Clinical Commissioning Group.

This service is run by Alzheimer’s Society. Referrals come from health professionals, or people can call the helpline number to refer themselves. It ensures that people get the support from local services to allow them to live well with memory loss and dementia.  For more information on support services in Dorset, please email: or telephone: 0300 123 1916.

There are 2 key documents that will be very useful for pharmacies e.g. during MURs/consultations: – the Memory Gateway Flowchart and the Memory Gateway Referral Form.
